Hello,
I’m curious if I need to bring my physical form for my pulse reading AMI. The only paperwork I see is the two papers that give directions for conducting the AMI, should there be any other paperwork for the nurses/doctor to fill out for the pulse reading or do they need to just input it into DODMERB?

Appreciate it sir. Apparently they had to cancel due to not having that paper and unsure what to exactly do. Both me and the nurses attempted to call the Civ Team and Remedial service but to no avail. So I’ll most likely have to reschedule another appointment, preferably closer however as the “nearest” office Civ recommends is 40 minutes away so a minute clinic would probably be more efficient.
