Reporting "Basic" Issues -- How Much Info/Detail Do They Really Want?

Going through the medical history with DS. Seems to me there are many things here that are going to be extremely common, probably even a majority of applicants will report, such as wisdom teeth extraction and/or braces.

For seemingly routine things such as this, when there were no issues or complications, is it sufficient to simply report dates of treatment/procedure and basic details, or are they going to want actual documents such as complete file from orthodontist or surgical notes from oral surgeon?

We've never done this before and just starting out, so hoping more experienced hands will have some advice. TIA!
 
I can answer the braces question. My DS checked "yes" to the question about ortho procedures. He listed the start date and end date (we called the ortho office to get the dates). He stated reason was routine orthodontic care. No paperwork was requested or provided.

The initial health questionnaire should be filled out ‘as asked’. I’m assuming they aren’t requesting a full file of paperwork related to wisdom teeth, at this point, for example. Later, additional medical information *may* be requested. Each case is an individualized look. So your question cannot be answered with certainty. They may, or may not, want what you are asking about.

When filling out anything, answer the question asked.

Thanks, that's exactly what I was looking for. I just kind of wanted to know if I had to dig all that stuff up prior to DS's exam. I can't imagine they really want to review records on all that stuff unless there was a serious problem.

As you fill out the questionnaire, anything that has a "yes" will likely lead you to fill out additional info, similar to the braces question. Looking at the general DQ list will give you an idea of they will ask for more information.
 
I recommend preemptively obtaining your entire medical history on a separate thumb drive for easy access.

Cross reference anything with exact dates.

When in doubt, disclose.

The days of “If there’s no scar & the doc won’t find it on a basic medical exam.” are over (I was on recruiting duty many years ago).
