It looks like they're scanning them the old fashioned way. It's two pages, and you can tell that it's stapled and folded over to scan the second page. Not very efficient so I bet it'll take awhile. -- Last name starts with D, in case it matters.

Available as of 4:40pm in uploaded files listed as other, from a fax, and in a .Tiff file format. I printed as a .pdf and saved it to a local drive.
