shoulder dq?

I am currently a candidate for the USMCR OCS. I am about to schedule my physicals and had a question about an injury I had in june. While bench pressing I hurt my shoulder and thought that I tore my tendon. I went to an orthopedic and he told me it wasn't torn, it only started to unravel. He told me to lay off any shoulder training for a month and I will heal completely. I did so and have not had any problems since. I have been able to continue bench pressing heavy without any issues. Since I went to get it checked out I know I have to mention it. Do I need to get some sort of letter or form from my orthopedic to bring with me? Also I am 30yrs old so I am in need of an age waiver if that makes a difference. Thanks in advance.
 
Include all the documentation that you have (keep copies for yourself, of course) when you send your info to DoDMERB. If they need further information they will ask you for it. DoDMERB has nothing to do with waivers, they only determine whether or not you fall within regulations. USMCR is the waiver authority. Best wishes.
