Has anyone actually seen a service academy SOE form email that was sent to a teacher? I just wanted to provide my teacher some foresight on how what sort of things the form said, since they already filled out my nomination recommendation forms, and wrote me regular rec letters and I didn't want them to think they had to start completely from scratch.
 
I haven't seen the SOE form but, the teachers who wrote your regular recommendation letters ( assuming you mean the 3 optional letters of recommendation for the academy??) will not be the same teachers who fill out the evaluation forms.

What exactly is an SOE form then? I thought it was supposed to be filled out by your teachers from junior and senior year?

It is not really a form as such. They will receive an email with a link to a secure web page where they fill out the evaluation and submit it to Admissions.

You will have 3 people fill out your evaluations. When you go to your application you will name your evaluators and generate a letter to give each of them that will have a code for them so they can fill out your evaluation. You can then send in ( optional) up to 3 letters of recommendation from 3 different people. As per the academy application :
While not required, you are highly encouraged to send in three letters of recommendation. These should be from people who know you well and who can provide insight into your character, leadership, and preparation for USAFA. They should not be from the three teachers who are completing our required teacher evaluations.
