When uploading documents to the application, should I include the prompt? What format- 12 point Times New Roman, single spaced?

I’d say don’t include the prompt; they know what they’ve asked you. I think your font selection is good, but double spaced as long as you can fit on one page, per instructions, tweaking margins as necessary.

"Include the Personal Statement number/prompt and your name at the top of each upload document. "

Each essay has a word limit, but no guidelines regarding font or spacing.
