Trouble locating old records

My son has been working to collect "all treatment records related to Asthma". We have heard back from 2 providers already that they don't have any of his records for him because it's been well over 10-15 years. What do people send if they aren't able to obtain these kinds of records? He's never been diagnosed with Asthma either, was just given an inhaler once by a small town doctor for a cough at age 4 which his regular doctor refilled at his yearly check ups to use when he had a cold. He hasn't used one for several years now, nor filled a RX for it since 2019, but it was after the 13 year old cut off, so we are going through the waiver process. He's passed the PFT easily and is waiting for his appointment with the pulmonologist.
 
I had the same issue. I know that at least in my state (MD) all records must keep kept on file with the corporate office as long as the patient is a minor. But I know mine was only for issues after the age of twelve.
 
We have an email sent to DodMerb with the same issue. My son was given an inhaler at age 8 because of allergies in the fall when he played football but never used it, so we have no records of any Asthma after age 13. I will let you know what the response is when we get it.

Did you submit a personal statement? A letter from your personal physician in addition to the specialist recommendation?

thanks! It seems like so many of the same questions get asked, I know there are swamped, I just wish there was a clearer path forward for some of this.

I just had to send an email to my doctor saying that I was applying to a service academy (the same as my pharmacy) and I had to make a few calls.
 
We had to see a Pulmonologist and do every type of Asthma test that they provide. They also requested his medical history after age 13 which doesn’t have any asthma cases in it, so we emailed DODMERB to find out what they want instead. We might go ahead and send in a personal statement explaining the situation

We are working on a letter from his current MD stating he has never been treated for Asthma under his care. He will probably need to write a personal statement for the time period prior to that, at least that's what we are thinking right now. He's just completed the pulmonary tests they requested and will see the Pulmonolgist in a couple weeks.

We requested his Doctor write a letter on his behalf to indicate that he never had Asthma under her care and we will turn that in along with it and keep are fingers crossed.
