Please include approximate dates, circumstances, symptoms experienced, treatment received, and response to treatment.

I just want some clarification on this, when it says "applicant's signed and dated personal statement regarding exposure history", does this mean that I will be writing my own report on my medical history or will I ask my family doctor to write this for me? Thank you

Only person that would be able to provide the correct response would be Mr. Mullen. I see you previously emailed him. I would email him again making sure to use the correct formatting and I know he will respond quickly as his record is well documented on this forum. Best of luck

A = NO. It is "YOUR" statement describing what was asked. Then sign; date it; and submit as a .pdf attachment.
