Where Do I find Tattoo and body fat forms?

I have my application complete and I am finishing my dodmerb exams, but I cannot figure out where to find the body fat and tattoo forms. I was told by a friend at the academy that you get the body fat test done at the dodmerb health exam, but I wasn't sure. Thank you!

The RC uploaded the body fat worksheet with a letter in the portal after the DODMERB exam was reviewed.

I got my body fat form in the portal. On the form it seems like there isn't even a place for my doctor to sign. Am I mistaken?

We thought the same thing and submitted it without signatures. It was rejected and we had to go back to the person (school nurse) who measurements and get her to sign it.
There is a place at the bottom of the form that you might think is for the academy to certify. It's actually for the doc/nurse.

Does everyone have to do these or do you only do them if they contact you? My DD does not have any tattoos and no one has requested a body fat worksheet.

Can't say for sure, but in the Army you only have to be "taped" if you weigh more than a number which is determined by your height and gender. This typically applies to those who are rather muscular, as well as the chubby folks of course.
I assume it's the same thing here. I never have to get taped and I have not received a body fat worksheet.
Just curious, is it the same DA 5500/5501 that we know and love?

Yes it is the same DA 5500/5501 that we know and love lol! DD always has to "tape" as she falls in the petite but muscular category (gymnast build). You will only receive one in your portal if your Dodmerb exam shows you fall outside of the standards for height/weight. Your RC will usually notify you if there is one required and then it will show up under correspondence section in portal @Bastogne36 and @KYparent
