We have been slowly requesting medical records from my son's doctors from when he was young (< 9 yrs) to see what's in them.

In one record, when he was 4 yrs old, the doctor wrote that he had symptoms of ADD. We don't remember any conversation with that doctor about our son having ADD (for many reasons, didn't stay with that doctor even a year). In fact, we don't remember having a conversation with ANY doctor about our son having ADD. He has never taken meds, never had an IEP, never needed accomodations, etc. He is taking a college prep courseload and juggles many extracurricular activities successfully.

Would he check "yes" on DoDMERB form?

And would this trigger an automatic remedial and/or DQ?

No, he was never diagnosed with it. There is nothing to disclose.

Sounds like the doc noted he might shows some signs - most four year old boys are easily distracted lol. Glad you switched docs :)

I also agree, symptoms of ADD is NOT a diagnosis of ADD. Do not check yes.
A yes will at a minimum generate a remedial if not a DQ for something your DS never had.
In addition, 4 years of age is very early for a ADHD diagnosis. WebMD has a good write up on diagnosing ADHD in preschoolers. It is very involved and difficult.
