Administrative AMI

I just received a letter saying that I need an Administrative AMI completed due to low BMI. The letter also states that because the AMI is listed as Administrative, that I can complete it on my own without seeing my physician. How do I go about doing that? Is there a form for me to fill out?

BMI-Body Mass Index? So “to complete it on your own,” you have to complete your own body mass index analysis and submit a letter. I can only relate to DS situation about a headache in the 7th grade. No need to go to a doc. He wrote a letter about the headache from 2016.

My son received a similar letter in January, however it was specifically from USMA not DoDMERB. The letter came with a form to fill out and submit. From the date of notification, he had about 3 weeks to get his BMI up and submit new data. After drinking protein milkshakes daily and having steak for dinner every night for 3 weeks (and after mom and dad each gained 15 lbs just watching this process), he went to a local Army recruiting office to be weighed and he met the requirement.

He did also reach out to our local USMA recruiter/contact for some guidance when he first received the letter. The form could have been filled out by his regular pediatrician, school nurse, Army recruiter, and possibly a few others. You can't fill it out yourself- someone else needed to certify and confirm the new numbers.

Curious- what is the BMI minimum threshold?
 
I had my physician write a letter saying "Weight is consistent and has been for years, shows sign of growth in height" or something along those lines. My BMI is 18.2 and I'm clear as of January (cleared for USAFA, underweight according to BMI charts seem to be under 18.5).
