Body Alteration Form Witness signature

Do I need a notary and all of that or can I just have my mom sign as witness.

I checked everywhere in my portal for the list of things they tell you to bring to I-Day, but I can’t find any of it. Do I receive this when I submit all paperwork? Does it come in the mail with my appointment? Thanks

... so a form with notary requirement should have a 4-inch space on the last page for the notary public to fill, sign, and stamp. If not, then you can sign it and your mom as a witness can sign it too.

Proof of Citizenship, Police Record Check, Body Alteration Form, Accession Screening Questionnaire, Medical Readiness Form. Due Date: ASAP. If you have not already done so, upload submit these documents via your CIS portal. This information is requested with offer of appointment via your candidate portal, not a part of Permit to Report Package.

I mean it looks like there is a perfect 4 inch space of blank for notary, but no where on the form does it say anything about notary. It just says witness signature.

Self-explanatory ---> Name, Date, Notary Number, Signature.

If there's a notary signature requirement, then go to a notary. Remember that the notary needs to eyeball you and the witness sign in person.

I would say you do not need a Notary. We will be using DM as the witness, and going with it. But I encourage all to make their own decisions.
