Checklist before BCT

Since this question is popping up regularly, quick checklist for all the lovely paperwork we have to do, when it's due, and what to do with it on I-Day.

* June 1st - Optometry order form (only if you need glasses for BCT); fax or email
* June 1st - Immunizations form; fax or email (keep a personal copy, as well)

June 7 - Sponsor Program preferences and information; mail, email, or fax
June 10th - Tattoo and Body Marks Form 4428, must be completed with an ALO; fax or email.
June 10th - Uniform Measurements; mail, email, or fax.
* June 11 - O-123 Emergency Medical Information - DO NOT SIGN; mail the original form and bring a copy.
* June 11 - SGLV 8286 Life Insurance - DO NOT SIGN; mail the original form and bring a copy.

??? Banking Agreement and Information - Sent these in and lost the instruction forms, so I don't have the deadlines for them (if you have them, post in comments). Must be mailed.

Other stuff:
* SF86 Security Clearance: You don't have to turn this in, just bring it completed to I-Day. According to information given by graduates and current cadets, you will be redoing the form using the information you're filling in now. Don't show up on I-Day and get screwed over because you can't remember your teacher's home phone number.

* Bring three copies of your travel orders to I-Day.

* If you're traveling on your own dime, bring travel voucher form, itinerary, and receipts

The General Questionnaire, Privacy Release, and Language Placement Test are due before I-Day, and are accessible through the email sent to you (Appointee Data Collection).

Useful Notes:
If you don't submit your immunization form by June 1st, don't panic - you'll just have to get them again during inprocessing (sorry, trypanophobes). Same goes for most of the other forms - for most of them, failure to submit by the deadline isn't an insta-fail state, but will complicate things for you. That said, GET IT DONE ON TIME.

For those wondering about the 7:00 AM arrival time on June 27th (in relation to squadron assignments), we'll be getting detailed arrival orders on June 12th (6:45, 7:15, 7:45...). If you're hoping to get a certain squadron for BCT, you'll know on the 12th.

Have decided to send cleats on I-Day just in case. After watching videos of what my DS is in store for at Jack's Valley, I figure carrying a little extra weight on I-Day is the least of his worries! :biggrin:
 
In the Checklist at the end of the Instructions to Appointees booklet, it says that all of the Required Items for Inprocessing should be hand-carried and shouldn't be left in our luggage. Can we not just keep some of these things in our bag? I understand some things, like the Photo ID, but what about the things like the deodorant and the shampoo?

I'm not class 2017. They want to make sure your paperwork is at hand. (Take a folder. Closed folder so nothing falls out as you go about the day and get yelled at :shake:)

Cleats

Hi
We just got back from buying cleats for my DS. We picked soccer cleats as they were very lightweight and since he will use them only as needed during BCT an inexpensive super lightweight pair was perfect. Some of the colors were guaranteed to get you noticed so he got the black with Air Force blue laces.

in our email that we received today it says, "This electronic medical packet is in lieu of receiving a mailed packet."

i want go get all the forms i got today done ASAP but do i need to wait for the packet in the mail or can I submit them all electronically? i think some say you need to bring a copy to i-day so i'd have to print those

From what I read it all needs to be filled out and signed and brought with you to I-Day. If not they will pull you from the line and make you fill it out there and it will slow down your processing time and maybe make you stand out????

Hey everyone,

All of the forms needed are in the email, so it seems. I agree with Ambition; it appears that the electronic "packet" is a substitute for the physical packet. Just by reading through the packet, it appears that the forms can be printed off, filled out, and submitted on I-day. The checklist comes in handy. For what it's worth, I was able to complete it all tonight (minus the skin test that I have to take). Pretty basic stuff!
 
I went through the same immunization form frustration last year. As long as you bring a copy of your original immunization records on I-day you should be all good. The sergeants there'll take care it from there - they know what to look out for.

the immunization record form that we had to submit to them already correct? i plan on having a couple of copies with me on i-day
 
If you had it filled out by your doctor, then yes - bring that USAFA immunization form and you'll be set.

In my case I had difficulty getting hold of my doctor, thus I filled out the form myself, and brought along a copy of my Californian immunization records.
 
To answer John's question about standing out, sort of. If you are significantly slowed down inprocessing (like I was when they had trouble drawing blood due to my problem with passing out when I get stabbed five different times in the arm) then you will be one of the last basics to get to your squadron. In some ways you might think this is good (less time with cadre) but I would say no. Once you are done with all your inprocessing, you will be setting up your room and doing inventory. That is a lot of work. If you are last to get there, you won't finish and will be stuck doing that when everyone else might have some free time or time to study or sleep. I barely had a chance to start before it was off to dinner and a bunch of other stuff. We got back just before TAPs and I had to set up my whole room. NOT FUN. Do everything you can to have all your forms and information with you and be well hydrated so you don't have problems with blood flow (I am talking peeing every hour minimum hydrated).

We have to give blood? I have small veins and very low blood pressure, so I have ALWAYS been a pain in the neck to draw from. This will be fun...

Definitely stay hydrated. That will help. The blood test is quite comprehensive. They test for a variety of things. My DS found out that he had a mild version of Glucose-6-phosphate dehydrogenase deficiency. He would never had known otherwise.
 
It isn't "giving blood" as it is much less than the pint usually donated, but yes, they take about 5 small vials and if you are unfortunate and get someone who isn't too skilled (like me) it can take a while :)
 
My 2014 DS also had the same experience with getting stuck multiple times. He still says that was the worst part of I day. He is now going to be a flight commander for 2nd beast-- can't believe how fast the time goes (for us, not him).
 
Not sure we understand.

DS is a Recruited Athlete in soccer, and has now Verbally Committed to AFA over the other SA's for 2014. We are proud of all his efforts, including that he is a strong student additionally. Mom is in charge of the family medical world and is unsure what he'd need in further shots as his record is complete and current. What else is the AFA looking for over and above what our Medical Professional has done to date for 17 years? Thank you.
 
Our DD is entering USAFA this summer and was completely up to date with her immunizations. However, she still had to have a TB test and a polio vaccine within 6 months of entering the academy. That's just one of their requirements! So if your DS is current then there is nothing for you to do at this point except wait for the appropriate time for to complete those requirements. When your DS receives his official appointment this will all be clearly explained in the paperwork.
