College Reapplicant Present Health Questionaire

I am not sure if I missed this someplace, but I searched and found nothing and browsed the different threads and could not find anything, so here goes...

DS has decided to re-apply this year, surprising us. Bottom line is he was found "Qualified" last cycle and his admissions reps this year and his DoDMERB rep all said all he needs to do is send in the "Present Health Questionnaire" basically stating nothing has changed or if anything changed what it is. The DoDMERB tech was helpful and said to fill it out per the Admissions people, but there is no guidance as to when it needs to be filled out and submitted. The DoDMERB Tech indicated to wait until he is told to submit it, but the admissions people were not as clear and told him to contact DoDMERB.

How does this work, should he send it in now that he has almost all of his applications complete or does he wait till someone asks him to fill it out and send it it. From experience, DoDMERB sometimes is not quick to request and I have no idea if the admissions people will ask him to do it or everyone will assume he did it. The horror stories on here tell me NOT to submit anything not asked for, but how will they ask him for this form or will they ask him for the form or expect him to know to send it in without being asked?

my DS was a re-applicant. as i recall, the present health questionnaire was sent to DODMERB, not admissions, and after sending it in his status changed to qualified. that wasn't until sometime in December.

the questionnaire is very simple, just a few check boxes saying you haven't had any changes in your health status. i THINK that you will be contacted by DODMERB and asked to sent it in. It's too early to worry about it at this point, remember, for re-applicants they won't review your package until first semester transcript is available.

my guess is that when a certain percentage of the application is done, the DODMERB actions are triggered. instead of being contacted by DODMERB to schedule the exams etc, you will be contacted to complete the form.

separate point - you said that "most of the application is done". does that mean essay and recommendations as well as activities record? if so, then it's done. but if not, don't rush. make sure that the fall college experience is reflected in the essays and activities, and also in the recommendations if possible
 
correct. my DS once he got so far in the reap, DODMERB sent the questionnaire. it was very basic
 
Thanks @A6E Dad, we know it gets sent to DoDMERB, he just can't figure out if he needs to wait for DoDMERB to request it or take the initiative and send it in. So often on her, he has read of horror stories of people sending in stuff without DoDMERB requesting it and then having problems he wanted to be sure. He spoke with the people writing his recommendation letters and has already sent admissions their info, is working on his essays, has completed his MOC NOM apps with all required info, has submitted his course schedule, and is working out in preparation for his CFE's. He was just getting a little confused as to how the DoDMERB update was requested or if it is not requested but expected for the candidate to know they have to submit it and when to submit it. He leaves for College Friday, and will be busy getting settled in and meeting professors, fellow students, and starting NROTC and classes and wanted to get done what he could before he leaves. I want to stay out of his way and let him navigate this by himself, but it seems like admissions and DoDMERB were not clear on whether to wait till it is requested or send it in.

Thanks...that makes it clear...I will advise him to wait till DoDMERB asks for it... And if does not hear from them by October to go ahead and submit it.
 
Bottom Line: For ROTC contracting, the DODMERB is valid for two years after the date of the exam (block 1 of the DD Form 2351).
