I've got a couple of questions about what exactly should be marked "Yes" on the medical history form, and though Mr. Mullen will likely just ask to have an email shot his way, I figure a few people may wonder about this.
First, alcohol is a choice under "have you ever or do you now use any of the following," and I think the last time, and really only time I had alcohol was champagne on New Years about 2 years ago (I don't even have it now for celebrations - really just don't care for it). Does that count?
Second, when I was in first or second grade I had a stye on my eye. Didn't last very long, but did go to the doc to get it checked out. Would I check Yes under Eye Trouble for that?
Third, this one's actually just a note to myself since basically what's here is what's going on the form. No need to address it
Fractured left pinky playing football in gym in third grade. Had a cast/splint placed on it. No deformities, no effects on performance.
This last one I'm curious about because it seems like everyone would answer Yes.
Had any injury or illness other than those already noted?
Does that include whenever I've been sick from a stomach ache/virus, etc? Or a sprained ankle/other minor injury where ice was the cure? I would assume that they mean something more serious, but the way it's asked seems pretty broad.
Also, my exams are in the last week of March when I'm off from school, since that's really the only time I have to do it (I'm either otherwise committed or have to wear contacts within the three day minimum before going for the eye exam, so that week is the only time). I'm just wondering if for any reason that will cause any timing conflicts with everything going through. For example, the Air Force doesn't officially give me the scholarship until the medical is good, and there's a deadline on when I can accept the scholarship offer, right? I'm not sure how that little process goes, so any input is useful.
