SO what happens if a Cadet gets all the way through 4 years and is ready to graduate but a week before graduation they are deemed unfit for commissioning. Would they be denied a their degree?

Not usually if it was something out of their control like a medical DQ. They generally get to finish out their 4 years if they're close to finishing and walk away without an obligation and diploma. If it was something of their own fault (honor violation, etc.), they would be held responsible for their actions appropriately. I had a couple classmates that were made into December graduates for doing stupid things during grad week.

Seen both of those happen as well. Generally if you're found on honor, that's the two options you'll end up seeing happen. The guys who became December graduates grad week were regulation violators, not honor.

Actually happen to a few this past year. Medical issue developed that prevented commissioning, but were allowed to graduate.

As per Google (USMA WB-4):
WB4 Washington Basement room 4 A room in the basement of Washington Hall which issues out all uniforms and equipment for USCC personnel.
