So for the first CVW form, Consent to Treatment of a Minor, do I sign it (the applicant) or does my dad sign it?

My daughter, who attends college, will let you have access to her information if you pay her college bills. Will that work for you? As a bonus, she can head to your house every few weekends so you can buy her groceries too. .....But wait there's more. If you act now, and call within the next 15 minutes, we will let you pay for her spring break, and she will send you text updates about how much fun she is having.If you are eighteen or older you will sign. Under eighteen (Minor) your parent will sign. I still get the tuition bills from school and the medical insurance premiums but unless my child allows it I can't see the college grades or under HIPPA any medical information (over eighteen)! Wonderful system, you pay the bills but are not allowed any information.
Well its actually the same thing at the Academy. You can only see their grades if a Mid signs up to allow it. Medical you won't see unless there is a severe issue and the Mid authorizes it. Only time my parents saw my grades was if I told them or showed them. So they never knew my grades. My parents only knew my medical after a surgery and our team doc called my parents to tell them I was doing okay because I asked him to. So not much different anywhere, except there is no tuition payment. But when it comes to spring break and extra spending money... yeap they will come asking!
They would call the MOC?!?! Just....wow.Yep. Privacy Act applies, depending on the mid's elections for release of academic, performance, conduct or medical info. As a BattO, when a parent called asking for info, I had to pull up MIDS (USNA mid info dbase) and determine what I could discuss. Never a pretty moment when mom or dad, possibly already irate about their understanding of a situation or lack of info, finds out DD or DS has elected "no." Many would then call their MOC, whose staffer would call me, and I would head for the support of the Commandant's JAG. Most parents were great. Some were not. Many times the whole thing could have been avoided if DD or DS had told DF or DM the entire truth in a timely manner.

I actually had one mom state quite proudly "I am a Cobra gunship mom when it comes to protecting my son."Taking helicoptering to a whole new level!!!
