DD 2807-1 Report of Medical History completed 2015

Is there a way to get a copy of the DD 2807-1 form that would have been completed in 2015 for admission to USMA?

Download Adobe Acrobat and then search for that same form online on Google. I was able to find it and open it with Acrobat.

You can send a signed letter to DODMERB requesting a copy of his completed form. You can also request his entire completed medical exam if needed.
