Is crooked teeth n issue during the medical exam. It's not bad but like would it be an abnormality?
 
Have you been diagnosed as having acne? If so, say yes. If not, say no. Do not check it just because grandma said you have acne.
 
FOR ALL - All questions are "(Do you) currently have or any history of?" There's a reason for every question asked and for every explanation an applicants provides. There are no "trick" questions, just an honest request to obtain your medical history

In this question it poses = Acne. ( "(Do you) currently have or any history of?" ) I'll defer from my colleague "kinnem." The questions posed is NOT, "Have you ever been diagnosed?" And that is for a very important reason also.

For folks that have/have had acne, it's a simple question = YES. Part two is to describe particulars of THEIR acne.

Thanks Mr.Mullen and also one last question regarding the Med History packet we print. Do the physician take the packet after the exam and do we need to bring anything to the eye exam other than ID? thanks
