Hello! I have my DoDMERB exams today, and I was hoping someone could tell me for sure what paperwork I need to bring along. I'm assuming I need to bring my medical records, the form the DoDMERB website had me print off at the end of the questionnaire, proof of insurance, and I.D. Am I missing anything?

I believe that is all my DD took. If you are under 18, you must have a parent with you.
 
Not sure if it's different in different areas of the country, but my son only needed his ID. I attended his eye appointment, but wasn't needed - I waited in the car in case. I didn't attend his medical, but was needed. However, they called me and I was able to email a signed consent after the appointment.
 
All my son needed was the form from DoDMERB, his ID, and me since he’s under 18. Insurance isn’t required as the government pays the bill and full medical records aren’t required unless requested, I believe. I suggest calling the office administering the exam and ask them. They will tell you exactly what to bring.
 
We called today to try and schedule the DoDMERB exam and the Dr. office was so insistent as to was going to pay for the exam they refused to schedule the appointment. I was able to make DS the vision appointment after they found him in the system. I guess I will try again on Monday.

When you call, you might tell them it is for ROTC. When we scheduled my son’s, they told us they “don’t do those kind of exams”. I called myself and the person said “we only do ROTC” I was like, yeah, it’s for that. It worked. Apparently some of these offices don’t understand what a service academy is!
