I was awarded LOA in October, got the nomination in December and accepted the offer mid December. I am still waiting for the physical offer by post. How long does it usually take? Should I do a follow up?

It will come. The fact of the matter is that you have accepted your offer. Spend your time doing something productive.
I imagine that after you accepted, your portal updated with at least three (3) new items that they need from you;

1. Police Record Check Form
2. Body Alteration Form
3. Proof of Citizenship

The police record check form is what slows some candidates down. Depending upon your age, there may be different rules where you live. Some agencies won't release data on someone until they attain the age of 18.
Depending upon where you live, you get this information from the State Police, County Sheriff, or local Police Department. In some cases, you can't request this information without showing up in person. You will have to check into it.

My DS is in the same boat. He emailed his USNA admissions contact and he basically echoed what THParent stated above. He said it should arrive in the next few weeks.

Before you send in the body alteration form and proof of citizenship form, do you have to wait for your appointment letter as a copy of that is requested to be sent in with the forms as well? My DS may have misread the instructions. At any rate, he has not yet received any mail relating to his appointment since accepting his appointment online back in mid-December.

There is no rush to get this in. If they say to wait for the latter, wait for the letter. There will be plenty of time to send it.

That's a new one. Last year, there was no requirement to send a copy of the Offer of Appointment. As our esteemed @kinnem suggests, do what they ask, when they ask.

There was nothing on my portal about sending a copy of the appointment letter with the forms. I accepted my offer last week, and the instructions on the portal just say to complete and send (by mail) the three forms.

(I have yet to receive anything in the mail about my USNA appointment)
