How to submit Transcript/College courses in High School.

Form 146 of Online Candidate Kit, asks:
Did you take any College Courses while in High School? (if so, you must submit a transcript)

How does one submit/request this transcript? There's no form for requesting this, like there is for the secondary schools.

Our DS took college courses in HS as part of jump start program, his college courses were on his HS transcript. Have you checked with your GC to see if they do this?
 
For my DS and DD, for any college classes they took in high school, they had to submit a transcript request directly to that college. Go to the college website, under Admissions there should be a link for a transcript request form.
