D257.00 - Refractive error greater than -1.00 diopters in any meridian
D257.20 - Astigmatism greater than 0.75 diopters
D257.60 - Unaided near visual acuity worse than 20/20 (not PPQ)

Everything you've listed are considered "soft disqualifications" for USAFA. There is no waiver needed. They break their medical qualifications into 3 categories, Potential Pilot Qualified, Potential Navigator Qualified and Commission Qualified. If everything else on the medical exam is good you will be found medically qualified, and Potential Navigator Qualified (PNQ).
