Letter draft for MOC nomination request

DS has accepted a spot as a sponsored prep this fall. All of the advice we have seen indicates he should contact MOC and complete nomination paperwork prior to reporting to prep school. Does anybody have a draft letter to get DS started?

One a related note, he fussed and fumed when I "strongly suggested" that he hand write a thank you note to each MOC thanking them for the nomination (he rec'd 3) but he did it. He apologized last week and said that was a good lesson for him to do the right thing just because it is the right thing and not because you have anything to gain. He said he is glad he carried through and whether or not it impacts a future nomination he can go into the process with confidence.
 
Go to the MOCs' websites and find the name, phone number and or email of their Service Academy Nomination staffer. It should have been on the info when he applied last year. Have him call them and let them know he is a reapplicant attending a sponsored prep school and ask if he has to resubmit the entire application or if they can re-use anything from last year. 2 of my son's 3 MOC's have a streamlined application for preppies. They don't have to resubmit LOR's, Essays, or photographs. They ask for a new application form be completed with new resume, new test scores if any, and final senior year transcripts. Your son's MOCs may have a similar process.
