Medical History- paper form or online?

With the packet I received from Concorde, there was a page that needed to be completed about my medical history, but there is also a form on the Concorde website with the same questions. Should I fill out both the paper and online forms or just one?

You can just fill out the online form, but I would print a copy of it for your records.

Just in case anyone else had the same question, do the online form. The online form takes all of the information given and submits it into a PDF document of the form that Concorde sends you, so that it's neat and typed. You then print it out and give it to the doctor at your physical.
