I'm currently working on my medical history survey. Sometimes during long car rides I get slightly nauseous. My parents have told me that this could be motion sickness. However, it has never been diagnosed. Is this something that I should mention even though it hasn't been diagnosed? Thanks.
 
Rule #1 of DODMERB: Do not self-diagnose. Leave that to the medical professionals. Always answer the questionnaire 100% truthfully, but don’t name an ailment for which you have not been trained to diagnose.
 
Rule #2, IMO, do this one with your parent sitting next to you. Making sure THEY also aren’t self diagnosing. Reason being, is you don’t know necessarily your own health history. As they have been in charge of it for most of your life. Plus, they are older, and more experienced at understanding the verbiage in the questions that ARE asked on the questionnaire.

And, lastly, because once this one is submitted, you cannot go back and edit your responses. Take your time, and answer the question AS ASKED. Don’t guess. Don’t infer. And see Rule #1…..DONT SELF DIAGNOSE (ie, sniffles aren’t allergies. Unless an allergist said they are).
 
Again.....A diagnosis is NOT what is being asked for. The specific questions are:

"Do you currently have or have a history of:"

My colleagues above are unaware of the folks THIS year and all PREVIOUS years, that were NOT admitted to ALL Service Academies with Failure to Disclose Conditions........They became medical turn-backs AT the Academy after arrival.
