Organizing USNA Permit to Report Package


For the files that give an error message when opening in Chrome or another browser - there is nothing wrong with the file itself, but it is formatted with text entry boxes in a way that Chrome does not read it well. If you download the file and open it with the Windows default .pdf reader, the Edge Browser, it still might not work.
One of the better free .pdf readers is Adobe Acrobat (download linked below). The "Fill and Sign" function in Acrobat allows you to fill in most of the forms on the computer and save yourself a lot of printing.

If you download Acrobat, make sure to uncheck the boxes offering to install McAfee software along with the reader.
 
Here is page 2 of the "Patient Registration Form" usma_2025 has organized as I.2.c. DS asked because he noticed the bottom of the form read "continued on back." But there was no "back" that he could find.

Uh oh um, I mailed it in with only the first page completed as that second page you attached was not on there… do you think USNA will be able to accept it without the back?
 
Why not overdo rather than underdo?
When you click on the Patient Registration form pdf on the USNA PTR website it only has the first page on there, at the bottom it said page 1/2 and it looks like @pnwpandas was able to find the second page. However, I have already put the form in the mail.
 
What did you send in for the “Mantoux Tuberculin Skin Test (PPD) Form”? My doctor just checked negative on the immunization questionnaire but didn’t give me additional documentation.

** Note: This is part of the checklist and is noted there.
 
Thank you. Just confirming I understand correctly, completing this box was the only documentation required? I had the PPD skin test done, not the QuantiFERON-TB Gold test, so there are no results to attach to the paperwork.
I agree with you that when reading the PTR CHECKLIST it seems they want something additional from the doctor regarding the PPD documentation because it is noted on the PTR CHECKLIST separate of the Immunization Record. So to cover all of the bases DS’s PCP printed him a PPD Placement/PPD Reading result letter and he attached it to his Immunization Record.

Thanks for the input. Just to be safe, I will probably be going back to the doctor’s office for the 3rd time to ask for a result letter. 😅

Found my answer in another questions strand on this forum :) This is the DBIDS card....for parents.
