Police Record Check Form Upload?

Hello,

In the initial group of forms we need to fill out after accepting an offer for USNA, there is a police record check form. It says to have all authority (Sheriff, police department) fill out and send the form in a stamped envelope. But there is a place to "upload the document" in the portal. Do we still upload a form even though we cannot fill it out completely ourselves?
 
You might do a quick search on the forum, this has been talked about a lot this cycle. I don't recall the specific answer to your question.

I both sent it and uploaded it. Not sure if that’s what you’re supposed to do but the instructions were confusing considering there is an upload button.
 
Do you need to get the form filled out by the police and sheriffs departments or just one or the other?
 
Here's what I did for mine: I went to the local (the city's) police station, filled out the form using my Admissions Counselor's contact info, informed my AC that I did so, and then once he received it he uploaded it to my portal. So check with your AC if you could do it like that.
