Present Health Questionnaire

Good Evening. I am a reapplicant for the class of 2026 of USNA. I was previously medically disqualified, but received a waiver. I am in the process of being reapproved by DoDMERB and am filling out the Present Health Questionnaire. I am in the same health as I was last year, however, earlier first semester I was diagnosed with mononucleosis. This illness did not affect my academics, but did affect my ability to participate in various athletic events through my NROTC unit. One of the questions on the questionnaire is "Since your DoDMERB physical exam, have you had any medical/activity/athletic restrictions greater than 2 weeks. I was on Light Duty for PT for about 2 weeks after I recovered from the illness (which lasted about 2 weeks). Since then, I was cleared by my physician and have normal blood tests. How should I include this in my paperwork? Will this be an issue for DoDMERB to medically clear me again? Thank you for your time.

Issue or not, you must answer truthfully.

My DS and his fellow Foundation Prep classmates have to submit the Present Health and Covid Questionnaires.

With DS’s medical waiver from last cycle, he notified USNA Admissions that he is a Foundation prep this cycle. His medical waiver was (re)approved the following day.

My best advice is to contact admissions and explain that you are a reapplicant with a med waiver last cycle. I’m not sure if you will be treated like a Foundation prep though and have your waiver reapproved.
