Sending in remedials

We finally have my son's remedials (allergies questionnaire and medical records age 10 to present) completed and ready to send in to DoDMERB. Quick question: Should he send a cover letter with the packet, and if so, what should it contain?

RetNavy can add to or subtract from this but this is what I did last year with my daughter -
we included a short cover letter, just stating contents and her full name and social security number.
I made a COPY of everything I sent. I sent it all USPS - priority mail with a delivery confirmation (only $.50 extra).
The information was processed very quickly after it was received as I recall - within a day or two.

When we sent in my son's allergy questionnaire and remedial medical history, we included a cover letter from my son that outlined what was enclosed and a short paragraph about how he is currently unaffected by the previously reported condition(s).

Print the label on-line at www.usps.com and use a Priority Mail envelope, and the delivery confirmation is free. :wink:
 
Both of those recommendations are perfect! It doesn't need to be anything fancy, just make sure you include what information is enclosed, and if you feel that additional statements are needed add those to the cover sheet.
