I think that is SF-86. The one OP is asking about is SF-600 which is a medical document that you have to fill out for the USAFA clinicAlso, at least for USNA, this is a pre-cursor to the actual SF you will be completing during plebe summer. Which is done online. I know both of my guys actually changed some of their references. I always advise against using BF/GF’s (and their parents)....even though at this stage you think it will be forever...and sure enough, one had initially used a GF on the forms (turns out mom was right, she dumped him during plebe summer).
Another hint, is to keep this one handy. A copy on your computer. It will be referred to a lot over the years, and beyond, depending on your job/clearance level. Or so I’m told....
