Just wondering, do teacher evaluations have to be from a teacher that actually taught you junior or senior year or can it just be a teacher at your school? Junior year I took Running Start classes and it was online so I didn’t get to know my professors well, and some of the teachers who would have taught me junior and senior year were my teachers sophomore and freshman year and could provide better and more clear evals. Also if I have taken physics and math from the same teacher, do I have to have two different teachers fill it out?

"The evaluation MUST be completed by the Math Instructor from 11th or 12th grade. "
I'm not sure if you're doing in person this school year but if you are try to make a good impression on the teachers in those areas ASAP. Do as much as you can, things like staying after to ask questions, participate in class, and if you're able to go into them for study hall (either getting help or offering to help others).
If you're not in person you want to try and do similar things online, one of my teachers would always start with me on discussions even if I didn't raise my hand because she felt that I was always willing to answer and like the consistency.

For the math\science problem id have them fill out the math evaluation then try and get someone like your counselor fill out the optional one. I know at my school the counselor was very involved with students and took special notice in getting to know me when she found out that I was going to try to apply to the academy. Other good options would be your chem teacher or JROTC instructor if youre participating in that.

I think what they're asking is if they were taught by a college professor over at college but it was as junior does that count.

Their teachers were from 11th grade and taught them while the student was in 11th grade so by usafa definition it fills that requirment. However it's still unclear if the teacher has to teacher a highschool class or can it just be a math while the student was in 11th/12th grade. I would ask usafa personally as this could be very tick for tac and they may only want a high school teacher ( but we don't know for sure).

Of course if not ask for one by a teacher who wass your 11th grade math teacher in the highschool just to be save.
 
I think you should ask your admissions team. My DS just sent his admissions team an email to ask about the evaluations since he is a homeschooled and I've been his only math teacher. It is clear in the Instructions to Candidates that if you have to deviate from the listed teacher, you need permission from your admissions team. His team responded to his email in 1-2 days giving him persmission to use a different teacher.
