vision appeal

When sending an appeal for vision DQ and waiver not approved, do you send the long verision form off the DODMERB site or just the short verision (eye examination data with boxes #17 - #23)
I am getting the packet together to send off this week and have both forms completed, but do not want to send to much or to little. Time is of the essence.
I corrected to 20/20!

Blocks #17 - 19 should suffice in your case. You are trying to get the corrected distant visual acuity DQ removed, so the only information needed is contained in blocks #17 - 19.
