What to bring?

I have my medical and eye exams coming up in the next two weeks, and I just wanted to know what I need to bring with me (besides my glasses and medical history packet). I have quite a bit of medical history that I noted in the packet, but should I bring documentation from my health care provider documenting any of the issues and showing that they were resolved?
 
I can only say what we elected to do last year. Our DS had previous surgeries and I thought they might be an issue. We obtained all of the records and the final statement by the surgeon and general physician stating that he was fully recovered and in good health. Those records we made a complete copy of and took to the medical exam. We also set a copy aside in case DODMERB would have asked for remedial info. Luckily, he was cleared without any remedial action required.

you dont need to bring your medical records with you, although we did have a complete copy just in case - they were not asked for. once the physical goes back to be processed, DoDMERB will notify you if you need to submit additional medical information. just bring the medical history paperwork you completed online, make sure your parent signs the correct page needed if you are under 18. If your under 18 your parent will need to be there to sign some additional papers too! hope that helps!
