CVS Pharmacy Records Request

Hello all! I won a 3-year AD Army ROTC scholarship, and, to become medically qualified (hopefully), I need to submit my pharmacy records from the past four years. My local pharmacy is CVS, and it has been recalcitrant to giving me my records. So far, I have faxed them thrice, created an account online, and recently sent a letter to ask for my records. The website only provides the past 36 months, and my faxes have received no answer.

Has anyone else faced this dilemma?
If so, how did you wrangle CVS into giving you those records?
 
Not sure, but maybe try actually going into your CVS in person with ID and ask them to print out your records. If not, your doctor may have records of the prescriptions he/she ordered for you. Doctors usually keep records of medications ordered in case any questions arise. Then you could give CVS dates and meds and ask for printouts.

CVS locations point-blank can't give out any information beyond 18 months. The pharmacist said that the information gets purged from the store records. I'm going to ask my doctors. Would records from the doctors be sufficient?


Additionally, I was not proscribed medication for the disqualification to which the request was attached. In the past four years, I was proscribed another medication that I listed on my physical, and amoxicillin (strep).
 
I would venture a guess that your Insurance Company has records of your meds since they pay for them. Not sure if 4 years are available online, but the insurance customer service line should be able to help you and get you the records you need. CVS and Walgreens, and other "around the corner pharmacies" bill your insurance, so your insurance company is the clearinghouse for prescription records. But it needs to be your Prescription Plan and not your Health insurer unless they are the same. Going back that far, online access likely isn't available, but the companies likely still have more than that available in their records.

Good Luck! And try not to panic, if you can only get two or three years send them with a written explanation that earlier records are not available, but a letter on letterhead from your doctor may help for records not available.

I literally just did this yesterday. There is a medical records release you can download for CVS and it has the fax number on it. Fax it to HX in Woonsocket, RI. Usually takes 30 days to get a response, I do this all the time in my work as a paralegal. Good luck!

Diue to HIPAA laws they need a signed authorization, because they can't verify you are really you on the phone.
