Do you need several forms/medical exams when you apply to several service academies?

I know that completing a medical exam is a requirement for attending a service academy. Do you need to complete several forms and have several exams done for each Service Academy, or could you just use one form for all?
 
For DoDMERB, you will complete your medical history and depending on your personal medical history, you may have to complete some additional specific questionnaires that are 1 page.

You can apply to all 5 Service Academies and all 3 ROTC Programs (to include the USMC Option of NROTC)...but ....WAIT FOR IT... you will only get one exam....One exam will apply to all your applications :wiggle:
 
@ABCDE_2026 you are asking a lot of good questions, but the fact of the matter is, they are all questions that have already been addressed in the information tabbed at the top of each forum. And it’s tabbed because it’s good information.

Additionally, go to the websites for USNA and NROTC. Research and read there.
Between the two places, every question you have asked is answered. If you read everything, and still have questions, ask away! But do some research first. It’s all laid out for you already.
