"YOU MUST complete the following tasks by the deadline posted on your portal:
Schedule your DODMERB medical examination. Admissions will normally not offer an appointment until your medical status is determined (See page 12).
Enter the names of your English, Math, and Other instructors for the School Officials’ Evaluation (USAFA Form 145). Regular/Reserve/National Guard Airmen have three evaluations to be completed by military personnel (Pages 8-9).
Complete the Candidate Activities Record (USAFA Form 147) and resume (Page 9).
Complete the Candidate Personal Data Record (USAFA Form 146) (Page 10). 6
Complete and submit the Drug and Alcohol Abuse Certificate (AF Form 2030). Regular/Reserve/National Guard Airmen must also submit a copy of their enlistment Drug and Alcohol Abuse Certificate (Pages 10-11).
Complete and submit the Writing Sample (USAFA Form 0-878) (Page 11).
Practice and Complete your Candidate Fitness Assessment (CFA) (Page 15).
The following tasks MUST be completed within two weeks after the deadline posted on your portal or your file will be closed.
Ensure your high school counselor has completed the Request for Secondary School Transcript (USAFA Form 148). Also ensure we have received an official transcript, school profile, and senior class schedule (Page 7-8).
Request official copies of your preparatory school and college transcripts (if applicable) be sent by your school to us. Grade reports are unofficial and are not normally used. If you are enrolled in college or preparatory school in the fall of 2020, your official fall semester transcript must reach us by January 15 (Page 8).
Ensure your English, Math, and Other Instructors (or Military Evaluators) have each completed and submitted the School Official’s Evaluation of Candidate (USAFA Form 145) (Page 8).
Ensure all ACT and/or SAT scores in your file are official."
