SOE's - How many do you need?

I've had three completed SOE's in my portal for some time, however, there is a red "X" under my third evaluation that reads "evaluation not on file: Add new below." What exactly does this mean? Do I need more than three SOE's for an application to USMA?

There are 4 required SOE's:

1. Math
2. English
3. Science
4. Physical Education

Science can only be physics or chemistry.

DS had 4 green boxes for SOEs. On review, USMA advised that a different teacher was required, and the box changed to red mid January. Once the teacher completed the SOE, the red box changed to green in about 3 working days.
