Various Forms

Can someone run down a list of the various forms our cadets might encounter with some frequency? For instance, I think Form 18 is for excusal from classes/duties due to illness? What's form 10?

Form 10 - disciplinary (good or bad) form
For 18 - medical form
SSS (Staff summary sheet) - special request form (travel, rooms, etc.)
Form 99 - request to leave the area (like traveling home)

NEVER forget the famous form 0-96.

This is the comment form used at every meal in Mitchell Hall.

The "traditional responses" to the six questions are:

FAST
NEAT
AVERAGE
FRIENDLY
GOOD
GOOD

How is a form 10 good or bad? What are the consequences for either?

You could do something really good like volunteer to help out in the squadron or at an event and be granted extra passes with the form 10. Or, you could do something really stupid and be granted demerits and confinements.

There is also the Form 34 to outprocess from the academy.

There are always those...uh...not so great basics to whom the cadre say something like "Why don't you quit? I'll even fill out your Form 34 myself!"

Now, now, Raimius... Perhaps that particular cadre member was having a bad day and just couldn't stand one more pouting basic!

As are wife and mother, there have been days when I was fairly certain *I* wanted to outprocess. haha
