What to do about possible atopic dermatitis diagnosis

Last year I went to a doctor and he diagnosed me and gave me a prescription but I soon stopped taking it when I saw it wasn't working. I then went on vacation to Spain and the pharmacist told me it was actually just Tinea Versicolor that got cleared up very fast with an anti-fungus cream but there is no paper trail for it. Do I need to state that I have had Atopic Dermatitis in my questionnaire or do I say I didn't have it and explain this to them when the prescription for it comes into question?
 
Well, you have a diagnosis. From a DR. And a prescription for it. And a question is asked about that diagnosis.

What do you think is the right answer?

There will be time to add additional information when/if the time comes. It’s called AMI. Dont be afraid of the process.

I assume you have or can obtain the prescription for the antifungal cream. In any case see the original doc so he can see it cleared up and document it. Bring the antifungal cream or prescription if you have it.
