I need to provide AMIs for my Asthma and ADHD, what do some of you recommend that I provide to them? I was thinking of providing a methacholine challenge test, and requesting my primary care physician to write or send a letter to the respective personnel. Other than those two what should I provide to DoDMERB, just looking for input from others? I also have stated that I am taking Singulair during my initial exam under asthma but it is for my allergies, how should I report this change.

There are forms associated with the AMI, it's in the dodmets portal under "Forms" (forgot what the menu option was). DS printed it out and requested his doc to fill-up and sign. For DS it was only one page.
It helped to read the explanation doc on the portal, written by Mr. Mullen (^_^). The whole process was very orderly for us after that.

Turns out that I don't need to provide a singular AMI (Orthodontic Questionnaire). And as Mr. Mullen said earlier in this thread, they know what they wanted and they request it, and they didn't request anything for my ADHD or Asthma yet. Reading and not jumping the gun would've helped me in this situation.
