How to apply for a ADD/ADHD waiver

That's going to be tough, @kaydeeloo. Waivers can be had, but the criteria to receive one are pretty clear, and you've missed a few of them. Would it be possible to schedule an ACT soon that he could take without accommodations? Not sure it would help, but I'd look for every bit of evidence in his favor.

Edit: out of curiosity, how did he get extended time on the ACT without an IEP or a 504?

Hmmm. I believe we are talking about the same thing. You need AMI. Same a remedial, right? So, you have to have to be able to satisfactorily answer those questions to get clearance from DoDMERB. If we had been asked for AMI, I would have approached the original provider and explained our situation and asked him to submit records and write a memo. I believe you can use your own provider for remedials, but you have to pay for them. Only difference with us, is that based on advice from this forum, I pre empted the AMI by providing the material up front- before they even had a chance to ask for it. Good luck to you and your son.

Background: I started taking medication (5mg of adderall) the beginning of my sophomore year. I stopped taking medication June 7th 2016 (end of junior year). That date was before my final tests, where I received exceptional grades. I also took an ACT and scored the same as I did while on medication(31). They requested my doctor provide my medical records as well as have my guidance counselor make a statement on any accommodations I had at school. Right now I am disqualified for history of ADD/ADHD as well as my eyesight(although that is a mistake). I finished my application in October and received a nomination, but I have not heard anything about a waiver.

My question is if the 12 months are the months leading up to the exam or the months leading up to the start of military service? Also, if you have any knowledge of a waiver granted that did not check EVERY single box, but most?
